These waivers will be available Monday morning when you drop your child off at camp, but just in case you want a head start, or just want to send them with your child, here they are. You can also sign them, scan them and send them to mbmcamp@gmail.com
These forms only need to be filled out once for the summer. The MBM Camp Waiver needs to be filled out for each participant, but the permission to bike home and photo release can have siblings on the same form.
